For seniors suffering from depression or with chronic mental illnesses, each day can be a challenge. At the Chelsea at East Brunswick, we recognize the importance of providing quality of life as well as ongoing support to the individuals, particularly during the critical transition period between hospital and home with our Mental Health Services.
Crossings provides an innovative, extended recovery program that combines all the services and benefits of a gracious assisted living residence with the skilled expertise of a clinical team to help those individuals who are managing chronic mental illness, depression, anxiety or have complex medication management needs. This unique transition option promotes the highest level of wellness and independence.
Crossings also provides special monitoring and supervision for seniors transitioning from an inpatient hospitalization through our Mental Health Services. Our goal is to continue the recovery process and to provide special attention to the elimination of risk factors that are key in the prevention of relapse. Our professional staff monitors and supports those individual areas of concern that are essential to each resident’s recovery including:
