The Chelsea at Fanwood is in the heart of the borough’s downtown and is not only Chelsea’s very first Assisted Living and Memory Care community, but it is also one of the very first in New Jersey. Since the time the community opened its doors in 1997, it has expanded to offer Independent Living, which is located in a separate building and adjacent to the original.
We are located off the main thoroughfare on South Avenue and can be accessed via Route 22.
Throughout its more than 25-year history, The Chelsea at Fanwood has provided seniors with first-class amenities and an enriching lifestyle. Additionally, our residents are noticeably healthier from a physical, mental, and spiritual outlook.

Annette is Executive Director of Chelsea’s flagship assisted living residence, The Chelsea at Fanwood, celebrating its 20th year in 2017. A building with a robust and distinguished history like this one deserves a leader with Annette’s deep compassion and experience.
Annette’s value to the company is reflected in her career here since 2004–she’s been Executive Director at three of our communities, always taking on a leadership role with a positive attitude and a caring way she wears like a badge.
A career nurse, Annette has worked with seniors since her time at the VA Medical Center in Lyons, NJ, and other assisted living companies. But it’s at Chelsea she found her home.
“We don’t just care for people, we care about people,” she says.
Maintaining a coherent and engaged staff is a top priority for Annette. And at the end of the day, she is proud of the Chelsea at Fanwood team.
Annette is a Certified Assisted Living Administrator (CALA) and a licensed practical nurse (LPN). She lives in nearby Plainfield and spends her non-work time gardening, dancing and spending time with her grandkids.
